The game is focused on Senua's (Melina Juergens) point of view, as she embarks on a very personal journey through a hellish underworld made up of Senua's psychotic manifestations of her reality and mind, all the while a dark rot growing on her arm, resulting in limited lives for the player. If the rot reaches her head, the game is over, and all progress is lost, forcing the player to start all over.
